Revision Rhinoplasty – Fixing a Bad Nose Job

Planning your secondary rhinoplasty

Each revision nose job begins with a consultation with Dr. Loeb himself – either in his New York City office or remotely (for patients coming from outside New York City). “The office has set up Skype or Facetime consultations, which can be used in conjunction with computer renditions to help plan your surgery and avoid extra trips to New York,” explains Dr. Loeb.

During this consultation, Dr. Loeb listens to the reasons a patient wants revision nose surgery. The patient discloses pertinent medical history information and expresses what he or she wants from the secondary rhinoplasty (or even tertiary rhinoplasty surgery). Dr. Loeb shares his opinion and discusses the best way to achieve the patient’s stated goals and outlines the steps for the suggested procedure(s). He also details the risks and rewards of such an operation. The planning stage is an opportunity for patients to have questions answered and develop a rapport with Dr. Loeb.

The reasons vary for desiring revision rhinoplasty, according to the American Society of Plastic Surgeons:

• 41% of patients required revision due to a new deformity that caused the nose to become crooked.
• 33% of patients received a rhinoplasty that failed to correct the original deformity.
• 15% of patients worried about loss of ethnic characteristics.
• 10% of patients were seeking further improvement for a clinically acceptable result.
• 1% of patients had trouble with airflow obstruction.

Overall, the study found 97 percent of patients are happy with the outcomes of revision rhinoplasty.

Once a patient has decided to have the revision done, Dr. Loeb recommends that he or she wait six months from the initial surgery to allow time to heal and soften. This is especially important for revision nose tip surgery, which the majority of revision patients request.

Revision Procedure

Dr. Loeb uses a closed technique, which reshapes the nose through internal incisions made through the nostrils, and:

• Preserves blood supply to the tip
• Allows faster healing
• Causes less swelling
• Avoids scarring under the tip

The surgery typically takes two hours in Dr. Loeb’s private suite on NYC’s Upper East Side. It is done with a light general anesthesia, protecting your airway at all times. The cost is similar to original nasal surgery. A few hours of recovery time on the premises is necessary before being discharged home. The details of the procedure will be discussed with patients at the time of their consultation.

Recovery

“Most patients are thrilled with the results they see at five days, when the splint and tapes are removed,” says Dr. Loeb. “They leave town or go back to work after that. Obviously, patients look better at 10 days when all bruising and even more swelling is usually gone. By 2 weeks, if you meet someone new, they probably would never know you just did something. By three months, you should feel 90% there!”

For a quicker recovery from revision nose surgery, Dr. Loeb recommends that patients:

• Eat a healthy diet and drink lots of water.
• Refrain from smoking.
• Avoid taking Advil, Aspirin, Motrin, and Vitamin E for a few weeks prior to surgery.
• Discuss any herbal or natural supplements being taken at the consultation.
• Avoid strenuous activities for at least two weeks.
• Try to sit up right as much as possible, including while you sleep.
• Don’t blow your nose for two weeks.
• Avoid swimming for a month.
• Avoid unprotected sun exposure for three months.
• Use contact lenses instead of glasses for the first four weeks after surgery.
• Try not to laugh too hard, smile too wide, or eat foods that require too much chewing.
